RETURNING HOME PROGRAM
Home Care packages designed to prevent readmission to hospitalization by offering homemaking aid and attendance during the first 24-72 hours after discharge from a Hospital or Rehabilitation Facility.
Reduce risk, prevent injury and promote safety in the home for a discharging patient through coordinating assistance to relieve the stress and tasks associated with acclimating to independence and ADL’s in the home.

Medication Reminder
Ride Home (up to 10 miles) from Discharge Facility and mileage rate apply for longer distances
LONGS PEAK Package:
Everything included in the MAROON BELLS Package, plus
Start a Couple Loads of Laundry
Wash the Dishes and/or Clean the Dishwasher
Shop for Groceries
Change Linens
Light Housekeeping Services
Prepare 2 Meals
PIKES PEAK Package:
Everything included in the LONGS PEAK Package, plus
Start the Newspaper and Mail Delivery
Pick up any New Prescriptions
Help you set up a Routine to continue your Recovery Based on Your Discharge Orders
Assist with Making a Follow-up Appointment with your Physician
Protective Oversight & Respite Care for Family Members
